Do not submitt the Conference Registration Form after September 27, 2010.
You may register on-site on September 28, 2010 from 7:00AM to noon at
the Conference Registration Desk.

How to Submit the Conference Registartion Form:

Download Conference Registration (Word2007) Form

Download Conference Registration (Word Old Versions) Form

Fill in all fields in the Form and save the file in any temporary directory.

Click on the "Submit Registration Form" link below

Prepare an e-mail message.

Attach the Word file previously saved and click "Send"

Cancellations - Refunds
Cancellations must be received by e-mail prior September 15, 10. Refunds for cancellations prior to September 15, 10 will be made following the Conference (less a processing fee $75). No refunds will be given for cancellation after September 15, 10, and an individual canceling after this data will be mailed a complete set of conference materials for which he/she is registered.
